Mucosal or rectal prolapse

I have a question. Suppose a person is suffering from rectal prolapse. The doctor is unaware of symptoms and history of patients. He just check his anus by asking him to stretch his butt and cough. If rectum does not prolapse and just moves a bit inside anus, can he still  make diagnosis that patient is suffering from prolapse.

Incisional Harnia and palvic adhesion

I have undergone laproscopic incidional harnia operation in 2014 with mash installed, from that time I was feeling palvic pain so in 2018 I have done CT scan of my lower abdomen and detected  as the uterus is adherant to theLSCS scar limiting the urinary bladder inferior to it. Few of the pelvic ileal loops are also adherent to anterior abdominal wall .Adhesions are seen between the uterus Android phone rectosigmoid.A small incisional hernia is seen in the midline in the infrared region containing a part of the pelvic ileal loop .Submucous fat deposition is seen in the colon and distal ileum. At present I feel some heaviness and tightness of my lower abdomen and somewhat stressing sensation in the cite of LS CS scar .Now from last few days I can see mucus like substance with urine and stool.I have done urine test there found urine infection,I find myself ok while using medicine but as I stop white mucus like substance again comes back with urine. Is all of these are side effects of mash
