Brain and Spine Problems

Hi, I am 31 years male. Since 10th standard have had a hardtime in education. Finally visited neuro in 2020 and was told have some white matter hyperintensities in brain which could cause focus and understanding. After joining family business, it got worse. Right now, I am on Comcerta and Antidepressant. I workout daily with moderate to vigorous exercise. Whom Can i consult to to address improve my understanding? Can someone help me with doctors who specialise in learning issues.

Went to neurologist and mri brain plain was done and report says " Focal area of encephalomalacia with gliosis is seen in right temporal lobe-sequalae of old insult" Pls advise if this is critical or can be cured. My father as usual whenever he laughs, he coughs, then automatically he becomes normal but today while same thing happened, he lost his consciousness, & it seems like he is dead. But he comes back to normal within few seconds, when we start shouting Papa! papa! Same thing like this happened 2 months before while taking his daily medicine, he swallowed the medicine with water & at sudden eyes gone up & it seems like he is dead, when we patted on his back, medicine came out and he became normal.. During this time my father consulted his heart doctor for this & the doctor said it was choking. . But today's problem is a bit different, today he is standing while we are making jokes. . I am 70 years old and currently taking Finecobal-NT for neuropathic pain. How long can I safely continue this medication and what are side effects ?

My mother is really having trouble in sleeping. She has thyroid for which she takes 50mg thyroxine and every six months it is tested which is also normal. She has migraine and takes tablets for migraine when its too severe. She takes early dinner and also walks in the night but still she says she just cant sleep.
