Minimal effusion in the right knee

It's been a month and I have been taking tablets almost 2 weeks now. But still a minimal effusion in the knee is not going down. I have taken MRI and diagnosed there is a small strain in the ligament. My problem here is why is the knee effusion is not going down even after taking tablets. But no pain at all in the knee. Knee injury

I had ACL tear recently and I opted for physiotherapy. My doctor asked me take 3 weeks bed rest and gave one leg immobilizer. If i set leg immobilizer tightly, then my leg will pain otherwise it will be loose. So is that pain normal? Also want to understand the purpose of leg immobilizer.
