Having pain when passing the stool and have piles. I have been taking ayurveda medicine but problem is still there when I stop using them

I have pain in last two days in near my ass hole. Last day I shows doctor she says this is first step of piles. So pls give me advise.

Hello, I am 24 year old bodybuilder suffering from a problem actually my stomach is not getting cleared from past 5 days and I also feel like I am going to poo but when I checked my self it seemed me that poo is still inside the rectum but when I pressure my self to do poo it doesn't come out which makes me feel a type of tightness in my stomach due to which I can't even eat properly and I have been missing my meals from 2 days it's getting worse for me day by day. please help out quickly.please
