Skin, Hair and Nail Problems

At the end of May I was bitten by a leech. In September skin on my calf started showing edema and infection and liquid is coming. Healing is very slow. Doctors that i showed to think that it is varicose. But there are no symptoms of varicose. One doctor gave me azithromycin and another aerofix antibiotics.

I am having a itchy patch on one of the side of my neck from last week or so and it is not going away. What could be the reason and treatment for this?

I had sun allergy for past 10 years. My B3 level is around 0.42. I took b3 pills [545 mg a day for 2 months] but it seems like my body is not absorbing it.. I had TB back in 1998 and i took isoniazid for 2 years which can hemper the absorption of B3. Fot the past 4 months, i started getting that necklace formation after getting sun exposure around 30 min. Do i need to get any specific tests done or do i need to take vitamin B3 at higher level..
