Kidney problem.

I have been sufferings from kidney disease since 1986.Now I am on the end stage I.e. stage 5.Dialysis not yet started.But creatinine goes up to 4.5, urea99,Uric acid8, sugar fasting 110. sodium pottasium normal.Urine out put normal.Slight swelling in leg.Breathing problem at night. Kindly suggest my diet plan.
