Please let me know what to do with this small tiny fissure .i am applying crema gel but goes and comes back .which cream is good for this and i am also eating softovac powder .for softening stool.yet the result are same. What should i do other than this please suggest me. I tried many doctors please me out.

I have swelling since 2020 and i was advised that my fiber intake is low and i should increase them in order for the swelling to heal naturally. But whenever I drink less water or having a busy day, I tend to have a hard stool with painful passing. And the swelling sometimes won't be there, sometimes it creates a hard times, like on and off I am facing this difficulty. Kindly help!
