Ringing in ear

I had ear infection 2 weeks ago..and after that I had crackling sound and bit of ringing in my ear and muffled voice. I have full day headache and sometimes dizziness when i suddenly turn or get up or go to a swing game. I don't know why this happens. I have done ct scan of my nose and attaching the reports .
