Is Als muscle twitching happen randomly? Or what are the main symptoms of als tell me about als pls 🤦

Dear Sir, I went to the Safdarjung hospital as my wife is getting unconscious, some times feels blackout. She has iron deficiency too. Following are the things adviced by the doctor. I'm unable to read it. Please help me with that. I can understand 3rd and 4th number point. Help me with the rest. And any other suggestions are welcome

Attached the MRI report along with this post. Most of the time, he is getting heavy pain and unable to talk or eat due to movement of the mouth. Few hospital are saying Go for a surgery or few saying , please stick with medicine. Generally, he is old and weak person and afraid of the impact of the operation. Need your suggestion what to do next Please help us with your opinion
