Brain and Spine Problems

I'm having some serious pain in my lower back from the past 1.5 years. I feel the pain when I bend forward or backward. Couldn't sit straight without back support. For the first few months I felt the pain into my leg upto knee but right now, I don't feel any pain due to sciatic nerve. Through some Google research, I found my symptoms match with slip disc. Is it better to consult a orthopaedic or neuro surgeon or a sports doctor? Can this problem be cured completely? Will I be able to play sport in the future after the treatment or surgery like before I had this pain?

I'm having sleep issue from a long time, Even if I sleep at 11 or 11:30pm, I wake up in the middle near 3:30am & can't go back to sleep. Sleeping issues are affecting my life especially my grades. I have Restless Leg Syndrome & anxiety issues

I am facing severe  migraine in left side of my head only. It is continuous from last 4 days and it responds to paracetamol but starts back again after 12 hrs of medicine. I feel like my head will explode. I don't have any past cases of headache this is my first time in my life am facing it. Kindly prescribe
