Bone & Muscle problems

I have pain under my both shoulder blade but most in right one and in my spine or upper back . I have also some lower back pain. When I squeeze my shoulder blade by connecting my both hand fingers towards my back I listen some crunching or crackling type sound coming from Upper back near my shoulder blade. I also listen this type of sound when I stretch my neck from side to side. Please note that I am also a patient of tmj disorder and the x ray of my neck region is totally ok . Please tell me what is causing my pain and how can I solve this problem? Who is the best initial doctor for me? Please Please help me.
