Tooth and Mouth Problems

I'm able to see a small black hole on right side of my tooth. from past few months.What could be the reason??

My wisdom tooth was hurting from yesterday morning....I went to a dentist...he gave me some medicine and a gel...when I take the medicine the pain subsides...but after 4-5 hrs...it comes back again...also this has resulted in fever as well...and shivering...the doctor said if the pain remains even after 3 days, then they might have to remove it....

How must cost will it take to repair one of my half broken upper front teeth as the original one tooth

These white patches are more like soft tissues and come of when pulled by teeth.they have been happening for 3 years.my mom also used to get them but it subsided after time.
