Tooth and Mouth Problems

I’ve got a really bad cracked tongue with cracks everywhere, it’s really visible and I’ve been too emabarassed to talk about it. I didn’t have it when I was really young and it started properly when I was around 12 and now I’m 18 and it’s really becoming a problem.

Yesterday night i had hash and weed and was chewing gum at the whole time. Is it harmful to chew gum while smoking?

Last year I noticed that my teeth are getting yellower at the root. So, I started brushing the teeth twice a day. But, it doesn't seem to help. There is also a yellowish plaque kind of layer behind a few of the tooth. This layer has actually removed the gums from a few places and it has actually weaken one of my tooth. I have also noticed blood in my mouth sometime, most likely due to him bleeding. What is this situation known as and how can it be cured?

I have been having ear pain for two weeks now I get frequent ear infections. My gum behind my left molar is swollen and has only been going on for 2 days now but I cannot eat or open my mouth fully without any pain.
