Skin, Hair and Nail Problems

Since many days my skin is itching and I can't even stay without scratching my skin. The more am scratching the more it is itching and rashes are appearing. Pls find the attached pic for idea of disease

Near about 5 years ago i have ringworm in the side area of my private part i got treatment and it was recovered that time but again it spred over my back side sitting area also again i got treatment and it was recoverd from last 5 years same thing happening i got treatment it will cure but when i stop treatment it  come again i want permenent cure
