Skin, Hair and Nail Problems

Can anyone please tell me the reason behind these bumps? The one near the nose is big. Why does it pop up? I have attached the images.

Hi, I am on very low dose accutane from long time. I also eat clean. But my triglycerides are high and and not in normal range its around 187 mg/dL I also  to drank coconut milk everyday ,could that be the reason? What can I do to normalise the levels? Should I be worried?
