Skin, Hair and Nail Problems

I'm not sure of the reason, but when I itch anywhere on my body, it is turning into a rash. They are like lines of itching and a few rashes around it. They itch a lot and they go away after a few minutes. It is all over my body wherever I scratch (stomach, arms, neck, ears, thighs, legs). I'm really concerned. What is the cause and solution?
