My son is 1 year 7 months old. He had one piece of wet tissue. That tissue approximately 6-7cm long. What needs to be done please suggest.

He has pain over both legs with swelling for nearly 1 week..and there is fluid like water in the swelling..on pressing wetness is felt..he is febrile..he has chillness..wat could be the reason?
