Back pain/cramp since last 2/2.5 years
I have been experiencing a peculiar lower back pain/cramping issue for the last 2–2.5 years.
During the day, I do not have any pain, stiffness, or discomfort, despite having a desk-based job. However, at night, after falling asleep around 10:00–10:15 PM and getting approximately 5–6 hours of deep sleep, I wake up with a severe cramp-like pain in my lower back.
When the pain occurs, it becomes difficult to turn from one side to another while lying in bed. I usually have to sit upright on the bed, and after about 15–20 minutes, the pain gradually subsides and I feel normal again.
This episode occurs almost every night and has been continuing for more than two years.
I consulted a doctor at AIG Hospitals, Gachibowli, who suggested that it might be related to Vitamin D deficiency. My Vitamin D levels have since improved, but the nighttime lower back cramping continues without any significant change.
I would like to know the possible cause of this persistent nighttime lower back pain.

Muscle weakness
I have been experiencing progressive muscle weakness, which initially started in my right hand, palm, forearm, and arm, but has now also started affecting my left side.
Symptoms include:
• Weakness in both palms, forearms, and arms.
• Burning sensation and muscle fatigue in the hands and arms, especially when holding objects for a longer period.
• Difficulty holding items for long due to weakness and burning discomfort.
• Inability to keep my arms raised for an extended time as they start feeling weak and tired.
• Weakness and burning sensation in my right calf and around the muscles near the knee/leg joints, which has now also started affecting my left leg.
• Burning sensation along the sides of both feet.
The symptoms seem to be progressing from one side to both sides and are affecting my daily activities.
Please let me know if there is any additional information you would like me to provide before the consultation.
Thank you.

Menstrual cycle problem
My last menstrual period was from 22nd to 25th May. Due to contraceptive failure, I took an Unwanted 72 pill on 27th May, within a few hours of the incident.
I experienced withdrawal bleeding starting on 2nd June, and it is still continuing. Until yesterday, I was having abdominal pain and leg pain, but since this morning, I have been experiencing significant breast tenderness why it's happening even after seven days of start of the bleeding, to the extent that even a light touch causes discomfort.
I am concerned about whether these symptoms are normal side effects of the emergency contraceptive pill and whether they indicate that the pill has pr hasn't worked successfully.
Additionally, could these symptoms be early signs of pregnancy, or are they more likely to be side effects of the emergency contraceptive pill? Should I take a pregnancy test, and if so, when would be the appropriate time to do so?

Fibromyalgia and related issues
My mother is suffering from fibromyalgia and arthritis-related mobility issues. She is unable to walk properly without support and her legs have become very thin , At night she experiences severe leg pain, tightness/cramping, and discomfort, especially after physical activity or household work. Medicines prescribed by the rheumatologist are not giving significant relief.
For temporary comfort, every night we press her legs using our own feet (pair se pair dabana/walking gently over the legs with feet) for long durations. However, she has become extremely sensitive to even slight changes in pressure or foot positioning. Even natural foot tilting on uneven surfaces while pressing seems to increase her pain suddenly. We want to know whether pressing legs using feet in this way can help temporarily or whether it may actually worsen fibromyalgia, muscle, nerve, or joint-related pain over time.
She also tried physiotherapy twice earlier but it did not help at all , now she doesn't trust it

Chronic Ear Pain and Severe Sound Sensit
I have been experiencing chronic ear pain and sound sensitivity for around 4 years. Loud sounds such as music, TV, mobile phone speakers, gym speakers, and other high-volume environments trigger significant ear pain and discomfort. Even normal phone speaker sound can cause pain and irritation.
Recently, after going to the gym for 2–3 days, loud music there worsened my symptoms, and now I feel a strong burning sensation inside my ears along with increased pain and sensitivity.
The pain feels like irritation and burning inside the ears, especially after exposure to loud sounds. This problem affects my daily life because I have to avoid speakers, loud environments, and sudden noise.
I want proper evaluation and treatment for this condition, as it has been persistent for a long time and seems to be getting worse after sound exposure.

Palpitations after insulin change
I having Type 1 diabetes and Hypothyroidism. For years, I was on premixed insulins (Novomix 50 and Mixtard 30) without these symptoms.
Three months ago, my regimen changed to Actrapid before meals and Xglar at night. Since then, I have new symptoms, mainly at night: breathlessness, fast heartbeat, internal restlessness/panic, throat discomfort, and disturbed sleep.
ECG and basic tests are normal. A doctor prescribed Fluoxetine, Etizolam, Propranolol, and Livas, but I have not started them yet.
My labs show: HbA1c 8.5%, fasting glucose 253 mg/dL, TSH 10.1 (high), low iron, and low HDL.
These symptoms began only after the insulin change. I want to know if this could be due to insulin timing/dose and glucose swings, thyroid imbalance, or iron deficiency rather than anxiety. Should I correct these first before starting the psychiatric medicines? Is adjusting insulin or returning to my previous regimen reasonable?

Nipple pain during breastfeeding
I am breastfeeding my 1.5-month-old baby (born at 34 weeks 2 days gestation) and experiencing persistent nipple pain and sensitivity. The pain is most noticeable at the start of feeds and sometimes continues during or after. Even outside feeds, my nipples are very sensitive to touch, clothing, or shower water. There are no visible cracks, wounds, discoloration and I don't think he has tongue-tie.
My baby is feeding well, gaining weight appropriately, and usually self-latches. However, I am unsure if the latch is optimal, as I continue to have discomfort. I have tried deep latch techniques (which sometimes worsen the pain) and nipple shields, but neither has helped significantly. My pediatrician suggested this is normal and will improve, but the pain seems to be increasing.
I would like guidance on whether this is a normal adjustment phase or if there could be an underlying issue, and how to manage or reduce this pain.

Hemorrhoids grade 1
I have had anal swelling for the past month. Initially it was mild but has increased over time. It started after a bike ride, followed by diarrhea and sudden swelling. A gastroenterologist prescribed IBIS tablets for my IBS and Anovate cream. Diarrhea stopped, but swelling increased. After that I went to proctologist who diagnosed grade 1 hemorrhoids and prescribed Anobliss cream, Benefex DS tablet 2 times a day and a stool softener at night. I am using these from last two days with sitz baths. Swelling reduces temporarily but returns after bowel movements. I have no pain or bleeding, only discomfort and heaviness in anal. Will this resolve permanently? Is surgery or sclerotherapy needed? Can I use ice packs after sitz baths for swelling? For reference I am attaching images here . . Sorry in advance for gross images.. Please suggest any treatment which will reduce this swelling to nill

Eye heaviness & difficulty keeping open
I have been having this problem for almost one year. I feel heaviness and tightness in my eyes with discomfort while opening them, as if my eyes don’t want to stay open.
Earlier it used to happen mainly when I was sitting idle, but now it has started happening even while I am working or active. The episodes come randomly sometimes for 1-5 min in a day , sometimes it remains for hrs and then stop then again happen..most noticeable for me is after bathing in morning..
When I feel heaviness in my eyes and then forcefully keep my eyes open for a long time, I start feeling head heaviness and dizziness. I also feel excessive daytime sleepiness.
There is no double vision. Eye check-up and CT scan were normal. My blood tests sugar, thyroid ,uric acid ,vitamins all are okay except for low d3 levels..Wind and cold water splash on my face gives some relief. The problem is not constant but keeps recurring somedays it's less somedays it keeps getting worse.
