Hi i took methotrexate injection for ectopic Beta hcg came down from 640 to 1.41 But the mass is not removed I want to knw whether surgery is needed or medication is enough

Past 5 months I dint get period and consult the gyno Dr they give me periods having tablet  for 5 days and then also I dint get periods and past 3 days heavy pain in spine center and passing through the under the rid and chest while I'm sleeping I can't breathe properly pain get severely and nausea and vomiting please reply what is the reason for heavy pain back and chest

My Beta hcg is 37.6 mIU/ml which was tested today noon around 2pm. My LMP is 28th april 2024. Today is 31st day. Can you please tell me if the values are fine.
