Kidney Problems

Dear doctor , My dad visited the clinic today for pain & doctor gave this prescription I have attached. Can you please explain me the meaning of what is written

I see foam in urine sometimes. Dr. Advised KUB ultrasound, UPCR and Kft. KUB WAS NORMAL All parameters in Kft are normal creatinine 1.04 and urea 27. UPCR .07. Pls advise i get worried when i see foam. I have read it is a sign of kidney damage.

All my fertility report are normal. What else should i do pls helpppp... .. . . . . . . . . . . . . . . . . . . . .

Having mild pain in upper back side (both side left and right).after the urination feeling uncomfortable in pelvic area some times feeling pain and mild burning sensation. Consulted with urologist doctor.he suggested to take scan ,CBC blood test,urinary analysis and urine culture test and everything is normal in the report. Taking antibiotics tablet since 10 days..but still I have the all symptoms I said above and no cure. What could be the reason of this issue???
