Hormonal Imbalances

My mother had suffered a heart attack on 25th of Feb. She got her treatment at Gandhi Memorial Hospital. She got her release on 1st March. Doctor said her heart condition is OK but the attack was due to high Blood Sugar.He replaced Januvia with istamet xr cp (100/1000) and asked to continue the insulin ( lupisulin 30:70 ) intake at 28/ 16 unit. But the PP results still coming around 477,387. Please help,it's urgent.

I am having blood sugar for last 10 years but it is always very high around 180 to 200 fasting I am regularly taking the medicine but it is not working
