Chest & Breathing Problems

My doctor suspects that i have ILD as i have acquired clubbing of fingers. But i don't feel any short of breath. I am a smoker. Please guide me whether my treatment is going on the right track. I am uploading the CT Scan report.

What is meant by inhomogeneous opacification in retrocardic region? Also the reports say Mild cardiomegaly and blunting of left costo-phrenic angle. Please assist me to understand this!
