Brain and Spine Problems

My right hand veins always swell and it aches near the wrist. Sometimes there is just swelling without pain, sometimes it pains and priks. If i pick something heavy imediately veins swell or if i get tensed veins swells automatically. why do veins swell? What treatment shall be done? If you can virw the picture you can see the swelled vein passing through the thumb.plz advice

Sometimes in the night after office, my left hand or right hand shakes uncontrollably(if I put it under my head last night while sleeping). Two times in the last two years my whole body shakes uncontrollably in the night time before dinner.I don't know whether it is due to excitation. I am scared of this situation. I don't know whether to visit a psychiatrist or a physiotherapist. Please reply me whom to visit.
