Kidney Problems

Yesterday I got my medical report in which it showed that my potassium and sodium level has increased  from the normal level. I haven't consulted with any doctor before. It was just a regular body check up in which I got the above results. What should I do in the given conditions?

Getting urine in Yellow colour done test like bilurubin and urine culture and i am not using any medicine ( as using the medicine may also give colouration).. No stomach pain. pleas help me
