Fasting blood sugar 138mg/dl. Urine 0.5(+) Post 247mg/dl. Urine 1.0(+ +). Can I have to take any medicine

Hi , My dad's sugar is shooting up till 440 in fasting where he only had morning tea n biscuits.. What could be done in this case , he is on sugar medication ... But I don't think that is working to lower lower down his sugar levels .. He use to regularly drink , smoke n chew tobacco.. and does not listen to anyone .. Any suggestions would be great help , though his treatment is on !!
