Hormonal Imbalances

My father 76 years has hypo thyroid presently taking thyronorm 25 mcg tablet since more than one year. in his recent blood test report T3 is 90.55 ng/dl ( range >50 yrs: 40----181 ng/dl) T4 is 4.7 ( range is>60 yrs: 5 ---10.7 ug/dl) TSH 20.9 ( range is 55 to 87 yrs:0.5--8.9 uIU/ml) so dose need to be changed or should be kept same . if changed than how much?

My Father is currently taking Triglynase 1 medicine for diabities and the same has been banned by Government now. Hence need a substitute medicine.

My diabetes range after meal is around 350 what should I do should I have tablets or go for insulin through injection.
