Suffering last 2months from breathing and disorder heart functioning and also weight loss

Hello doc, I am suffering from Good Lv function with Grade 1 diastolic dysfunction with sinus tachycardia, I feel sometime chest pain, shortness of breath, swelling on my left feet and sometimes i feel panic attacks also. Feeling pain at heavy heart, and getting little bit pain some times and backside of the left shoulder.

Earlier today a collapsed in so much pain in my chest, I went completely blind for about 4 minutes and my head was pounding from a massive migraine. I couldn't breathe no matter how hard I tried and my heart was racing. My friends fed me bread and I felt a little better but I still have anxiety and really bad anxiety about what could happen/did happen to me.
