Ear-Nose-Throat Problems

I live in a hill station so when ever I travel from lower altitude to higher altitude area, my right ear starts to pain a lot. One time it was so intense that some blood came out from the ear. So I consulted some doctors and some suggested me to go for operation and one doctor says that i can go for Gromet insertion as the ear drum is damaged and air circulation is not proper in my right ear. Other than that he told me that there is no tumor nor an infection so nothing to worry. For the fear of facial paralysis and my job I have been staying away from travelling to high altitude places. I am fine if i don't travel and there is no pain. But whenever i visit to a hilly region, I tend to chew gum and  do some breathing therapy and it does help to reduce pain.Well its been more than 4 years. I don't know what to do. Is there any risk?

Having headache and nose blocked badly. I bought a nasal spray ( BUDENIDE) . I just wanna know how long can i use this spray... Does this nasal spray cause rebound effect.? Can I use it twice a week?

I am suffering from positional vertigo since one week. It happens when I turn my neck to right when I am lying on my bed or when I wake up from my bed or sometimes when I look down/ up.
