Chest & Breathing Problems

I have always chronic cough in throat and now coughing blood in the morning i don't have any other problem just cold and coughing are the main things

I am having chest pain and shoulder pain since 5 months. Sometimes i feel sharp pain in chest and shoulder ,sometimes in the next also. Having blockage issue just below the ear.

X-RAY Chest PA report: Mild cardiomegaly, Prominent broncho vascular markings noted. Suffering from Asthma for past 20 years

I had this massive pain in my chest, precisely in a point above my heart. In the beginning it was unnoticeable but now ( after like 5 days ) it hurts so much that i cant laugh, sneeze, cough, clean my nose, without getting hurt. When i look at my chest in the mirror, i can see that there's a swelling where it hurts, and my mom was applying a cream she noticed that this swelling was not soft from the inside. It was very hard and the moment she touched it it hurt me so much. So now i kind of got used to this pain. I have done a blood test ( a doctor recommended one ) and i realised that mostly everything was good except for 'monocyte' ( the avergae was 5-10 and I had 16 ) I'm 17, very actif ( well i used to before this issue ), a bit asthmatic.

I'm post operated gynecomastia patient. I had done surgery of both side chest in 2012. My right chest is fine but my left chest bigger than right. And when I'm accessing it get swell more. I feel embarrassed. Does it means I still have gynecomastia. Please help. I need treatment of this problem.
