I have sujhan in my molar tooth and was bleeding. its so paining can you suggest whats the issue

I am having hardened dental plaque in my lower jaw's teeth. I have been suffering this from past two years. In every 2 to 3 months I used to clean. It hurts more now a days after having food. Please suggest me here.

If i brush at night too then it gets removed. why is this white layer getting formed?
