Due yo any emotionsl thought. .our future problems i got a attack like situation.. vomit high head ache. i cant understand what happend to me

I am DVM and MSc and employed for 4 years. My birthday is 11 Nov 1987. Last October I unconsciously write my birthday instead of actual date on cheque which I used to give to my father from last 4 years. My father told me about after bank made an objection. But on 17 Nov I wrote another cheque to my father and he reminded me to write date correctly. But again i unconsciously wrote the my birth year on the cheque for the second time. Again this mistake is being noticed by my father.

I'm thinking more constantly worrying lost interest in studying daily to do activities just want to sleep more and more
