48 yrs old male pt having Creatinine 4 since one yr and taking Ayurvedic medication, it has drop down to 3 and gain increased to 4.pt is known diabetic and hypertensive since 10 yrs. Can we control by conservative medication or do we need dialysis?

We have done routine urine test of my mother as prescribed by my family doctor, the report shows excess protein in urine, previous two reports also showed excess protein in urine.
