I have tsh level 24.how much dose can I take daily. I m suffering with back pain & neck pain...Plz suggest

My mother had suffered a heart attack on 25th of Feb. She got her treatment at Gandhi Memorial Hospital. She got her release on 1st March. Doctor said her heart condition is OK but the attack was due to high Blood Sugar.He replaced Januvia with istamet xr cp (100/1000) and asked to continue the insulin ( lupisulin 30:70 ) intake at 28/ 16 unit. But the PP results still coming around 477,387. Please help,it's urgent.
