My baby of seven months (will be eight months by April 1),recently started pooing immediately he feeds. He was on exclusive breast milk for six months. He hasn't totally adjusted to formula when i introduced him to formula after his first six months of life. He poos now immediately after feeding especially after being breastfed. Please what is happening?

10 year old son has had a rash for nearly 2weeks now that had started on his hands that he kept scratching and is now appearing on his legs and chest .
