Chest & Breathing Problems

Multiple ring enhancing lesions .left hand and left mouth becoming numb. vomitting, Extreme loss of  weight and investigation reported miliary mottling on chest x -ray . What is the next step to be taken ?

My father who is 66yrs old was a chain smoker and drinker for more than 30 years. He quit drinking about 3 years ago and quit smoking in oct 2016 after a mild attack. Since start of Jan 2017, he has been admitted twice in hospital for breathing problem and severe cough. Hospitalization has given him relief for a month or at the max and he is back to coughing.
