Skin, Hair and Nail Problems

Last year I had itchy red spots all over my legs and hands which was diagnosed as Tinea by the dermatologist I visited. It got cured after a year and reappeared later on in the same spots, which took some time but got cured. From then on I have had those red itchy spots appear only on the butt area which I think maybe tinea as well. But the same medications, cream and powder which was prescribed to me aren't showing any effect and instead is spreading towards the back and the skin is majorly scarred due to itching. It's been 8 months now. Due to covid situations unable to visit any doctor physically. Could you please help me out with this situation. Thank You.

Boil/pimple type developed on right inner thigh. It is very painful and can't walk as it can rub to other thigh . The size has increased since morning. Been there for 2 days. From image you can see the surround ing has been swollen a lot. What is it exactly and Is there an immediate relief? Maybe applying ice or heat? Some sort of cream?
