Small bump on eye like Granuloma

Here’s the short, corrected version: --- I had pterygium surgery 1 month ago. After 1 week, a small bump appeared where the pterygium was removed.  It shrinks little when I use steroid ointment, but comes back after in its original shape. My doctor suggested using steroid eye drops for another month. Should I wait, or will I need surgery again?  If surgery is needed, will it require autografting like the first pterygium surgery?

About 7–8 hours ago, my fingernail accidentally scratched my eye. After that, I noticed two small red spots on the white part of my eye. There is a slight burning sensation, but I have no vision problem and no severe pain.
