Doctor, I’ve been dealing with a distressing issue that I’m struggling to understand, and I’m not sure if it’s physical or psychological. As an adult, I am experiencing involuntary bedwetting whenever I fall asleep at night. For some reason, my brain just doesn't get the message about urine at night, and I don't feel the sensation of needing to urinate while I'm sleeping. Because of this, I've developed a strong anxiety around bedtime, and I actively try to stay awake at night to avoid sleeping just so it won't happen. It's affecting me every single day, and I'd like to figure out what might be causing this—whether it's an underlying physical condition with my bladder, nerves, or sleep, or if it's connected to stress—and what we can do to treat it.
I recently had a semen analysis showing a sperm concentration of 45 million/mL, progressive motility of 25%, and normal morphology of 1%. I would appreciate your advice on what these results mean and guidance on the next steps. What could be the possible reasons and solution
I have taut bands or taut muscle in pelvic floor
Mainly levator ani, puborectalis muscle. Left and right both. More towards 5 to 8 clock. Psyio not helping much. I hv pulled this muscle aggressively during my sons vaginal birth. I have problem in passing urine nd stools. Rest all test are normal. I feel something moving in pelvic. Muscles don't feel soft. It feels foreign like steel, metal. Folded. I just keeping pushing it down unconsciously nd releasing it. I am comfortable doing daily activities, wearing tight clothes or sit comfortably. Pelvic don't feel normal like before . Pls suggest whether I shuld go for dry needling, or botox or tpi as I don't hv much pain.
Iam having 47cc bph. Taking Silodal d8 tablet daily 0ne after dinner for past 4 months on doctor advice. My query shall i continue or go for turp surgery. If so which surgery is best. I heard after Turp retro ejaculation problem will be there.

My father aged 74 has been diagnosed with Echogenic nodule in his prostate. His prostate size being 98 gm. He got an ultrasound done with complaint of burning while passing urine only when he has to hold passing urine for a while..His PSA is normal at 0.6, Urine R/M report is also fully normal. Revered Urologists here 🙏🏻 is Echogenic nodule a cause of concern?
