I am a 37-year-old male with Vata–Pitta prakruti want Ayurvedic guidance.
For the past 2 years, I have had chronic bloating and excessive burping, along with weight loss of about 10 kg. In August, I experienced breathlessness in the daytime and occasional palpitations at night. ECG, 2D Echo, and TMT were done and normal.
Then, I underwent
Basti in October
and Virechan on the 18th of December
Post Panchakarma, my bloating and burping improved.
After Virechan, my doctor prescribed Sutshekhar Ras Gold (morning and evening, 30 minutes before meals). After taking the first day’s dose on 23rd December, I experienced sudden night-time breathlessness around 4 am, lasting about 1–1.5 hours. This was the first time breathlessness occurred at night. I stopped the medicine after this and symptoms disappeared. would like expert opinion on:
Could Ras aushadhi trigger such symptoms post Virechan? is Ras medicine necessary post Virechan? what should be next approach?

Radiologist:
There is a large midline complex mass lesion in the pelvis showing regions of T2 hyperintensity and intermediate signal intensity, with heterogeneous irregular frond like T2 hypointense regions in between. Part of lesions appear hyperintense on T1 and T1 fat saturated sequences. Patchy diffusion restriction noted in the T2 hypointense region. Approximate measurement is 9.1 x 4.7 x 8 cm. Post-contrast study shows mild septal enhancement, with subtle patchy enhancement noted in T2 hypointense region
The above lesion is adherent to the uterus.
Ovaries are not seen separately.
Impressions:
Pelvic mass is concern for endometrioma, as detailed above, with mitotic features. D/D mucinous cystadenocarcinoma. Ovaries are not seen separately.
Focal tiny hypointensity noted in posterior wall of uterus, in the region of pouch of Douglas, concern for adhesion due to endometriosis.

Is such a complex condition treatable in Ayurveda ?
My symptoms:
1) Unnatural burping and farting (indigestion)
2) Weight loss, from 71 to 47 in the span of 8 years.
3) All the blood reports, including colonoscopy, are normal.
4) Sucked-in, abnormal, deconstructed, heavy, hard or extreme loos feeling in the stomach.
5) Restricted downward movement (Apan Vayu). It's not average constipation. Stool quality is not proper, bulk is missing. No loose motions at all.
6) Also, secondary Pitta imbalance along with Vata. (Shatavari or Kamdudha Rasa tablet give me a good feeling in stomach, but it's a self medication). Slight usage of ghee makes my stomach feel normal.
7) I usually take Hingwashtak, but it increases heat and my sleep becomes fragmented and broken. Acid/pitta controller medicines give me a good sleep dramatically. But that Churna still goes ok with my system.
8) I get hungry and sometime not. Although I never lost interest in food, I get hungry too. But I can't gain weight because of the above issues.
