Diet plan needed

Hi, i have done my medical checkup in which below results came abnormal. I checked on internet it is given maximum abnormality can be corrected through diet and exercise. I started doing exercise daily but need diet plan for resolving abnormal issues.

Is it advisable to give any multivitamin, immunity booster, or health tonic at this age? 2. Are supplements like Vitamin C, Zinc, or any pediatric immunity syrups recommended? 3. Is there anything else we should do proactively to help strengthen their immunity naturally? 4. Are there any signs of nutritional deficiency we should watch out for before considering supplements? Doctor, I'm 18 years old and currently weigh only 38 kg. I'm trying to gain weight in a healthy way because but I fall sick very easily. I recently had cold, throat pain, and fever.
