Painful Acne - isotretinoin Advice

Hi Doctors , I have acne for more then 10yrs now. Earlier i had pcos. But i have checked recently my pcos is cured usg pelvis and total testosterone test came to be normal with no medication just diet control. I have been on doxycycline for 3 months for my acne. But it came back after 6month of acne free skin. Now am prescribed with isotretinoin 20mg. Looking at the list of side effects am a bit concerned to take it or not. Please suggest. I have attached a pic for your reference.

So the question here is can a diabetic patient use a derma roller of 0.5mm for the hair treatment. Is it safe?
