Hello Doctor,
We would like to consult you regarding this critical case. The patient (65-year-old male) is currently admitted at Max Hospital, Dehradun, and is on tracheostomy support following severe pneumonia with sepsis.
Despite ongoing treatment, there has been no significant improvement. The reports also indicate a multidrug-resistant infection (Acinetobacter), and the patient is still dependent on suction and NG feeding due to swallowing difficulty.
- Current line of management
- Possible changes in treatment
- Prognosis and expected recovery
We would really appreciate your expert opinion and assurance on how to proceed further.
Thank you.

So yesterday I felt sudden dizziness and subtle imbalance. Had to rest after that, whole night wasn't so active like always. Then the next morning it felt like heaviness on the head and imbalance, unsteady while walking or movement. And 2 days back I got excessive sweating while working. So need to know is this happening due to heat or anything like that?
My 78-year-old grand father had recent hip surgery 15 days ago
Initially (few days after surgery), he had:
confusion and delirium which improved after we started melatonin 3mg.
But for the last 3 days:
during daytime he is mostly normal
At night (after ~9 PM), he becomes very restless:
keeps turning left - straight continuously
cannot stay in one position even for a few seconds
asks to sit every 10–15 minutes, sits briefly, then lies down and repeats
appears sleepy but unable to fall asleep
There are no hallucinations now, but restlessness is severe.
His reports show low sodium (129).
His sugar in today morning - 78
2 hours After lunch (just 2 spoon) - 90
He is also eating very little. He doesn't like taste of anything at all. No juice, no food nothing. He's hardly eating 2-3 spoon. So somehow I started giving him ORS twice a day.
And yes He's very stubborn person - doesn't like to go or stay in hospitals. even for broken bone surgery we had to convince him too much.
Patient is an 88-year-old Female with severe anemia (Hb: 7.0 g/dL). Doctor has prescribed Iron Sucrose (Irozorb-S) IV injection. We live in a remote location, making hospital transport very difficult. Is it safe to administer this injection at home given her age? We are extremely concerned about the risk of Anaphylaxis (severe reaction). What emergency precautions or medicines (like Adrenaline) are mandatory? Also, are there any high-absorption oral alternatives (like Liposomal Iron) that can raise Hb levels quickly for an 88-year-old?
