Fever - How bad it is for our Body. Do's and Don'ts of Fever.Once at evening an anxious mother called me for her daughter, who has consulted me on previous evening for acute upper respiratory tract infections. She was worried that her daughter's fever (around 100 F) has not reduced although she has started eating well, playing games, studying and also she has now a very little coughing then previous day. This is what called as #Feverphobia.Dr. Barton D. Schmitt,M.D. was the first to give this term. Then studies were conducted many times and each time it is observed that most of the parents have excessive, sometimes unrealistic fear for fevers, the FEVER PHOBIA. They were worried that fever can do damage to brain or other parts of child, and they considered fever as the sign of prognosis which must be treated first..Actually fever is normal fighting mechanism of body's defence system. Fever itself is not a disease but it's a sign of underlying disease like various viral / bacterial / parasitic infection , any injury of inflammation in body , some adverse effects of drug , cancer , etc. So importance should be given to the underlying cause , associated symptoms and general condition of the patient. It has been observed in many times that a child is feeling week, dizzy and dull at even 100 F but other is playing and eating well even at 104 F.        # Body temperature below 100 F is not actually fever. Hypothalamus is a centre in our brain which maintains body temperature, it increases temperature whenever required. Rise in body temperature may be due to physical exercise or work , heat , stress, infection or any other diseases, too. Sometimes feeling warm or heat of body after playing or exertion, in child, is considered as fever and given unrealistic and unwanted medicines.     # If you treat fever first and underlying cause like virus or bacteria remains inside the body, you may actually harm your child by two ways,                 * first you are allowing those agents to grow by decreasing body temperature and                  * second you are suppressing natural reflex of your defence system.    Many times fever medicines are given or taken by self without consulting any doctor and this is the worst thing to do. Sometimes while again consulting the doctor fever is given the first importance to be treated is again worse.First consult the doctor, diagnose the cause and take medicine accordingly to the cause. But the first thing you should expect is relief in general condition of patient and underlying infection's symptoms  , not the fever. Patient should first recover in weakness, appetite, cold-cough or any other symptoms of infection or the cause of fever. Febrile convulsions which are also been the concern is very rare and are not so much harmful as they are assumed.Conclusion is we should come out of our myths regarding fever which in most of the case is a helping hand to fight the diseases.Role of Homeopathy: Homeopathy's main idea is about increasing immunity and defence mechanism of the patient. Don't Fear Chikungunya - Here's all you need to know!

Chikungunya fever is neither deadly nor contagious. There is absolutely nothing to fear.What is Chikungunya?Chikungunya is transmitted by mosquitoes, which breed in collected water. Direct transmission from one human to another has not been reported so far. Usually multiple members of a family fall sick at the same time because the transmitting mosquito is like a devil with a huge appetite for blood and cannot fly for more than 100 meters. In most cases, same mosquito bites multiple members of a family.After the bite of an infected mosquito, onset of illness occurs usually between 4 and 8 days but can range from 2 to 12 days.Chikungunya is “not” fatal Yes. You read that right. Chikungunya infection does not kill people. What is really fatal is the heavy antibiotics dosage prescribed by the quacks. They often prescribe 2-4 antibiotics at a time, which is too heavy for the body to handle and eventually leads to multi-organ failure.A body that is struck with fever is unable to digest food, let alone “digest” and metabolize heavy antibiotics, especially if they are the kind that are given after a heart/bone replacement surgeries or fractures.They are required in such treatments because the exposure of operated tissues is much more to the bacteria present in the environment. They also help avoid any infections being picked from the hospital environment.Symptoms of ChikungunyaThe symptoms of Chikungunya appear between two to twelve days after the mosquito bite.High fever and headacheIntense pain in the joints (ankles, wrists) that can persist for several weeks.The joints of the extremities in particular become swollen and painful to touch.A rash may sometimes occur.Haemorrhage is rare, and all but a few patients recover within three to five days.Some patients can suffer from joint pain for months even after recovery.Lifestyle advice for treatment of ChikungunyaComplete bed rest for at least 7 daysDrink lots of water, at least 5 litres per day for a normal adult. Plain water leads to distention  and too much glucose leads to cramps in the food pipe. Water has to be given indirectly by ORS (sachets or home made with lemon juice plus spoon of sugar and pinch of salt in per litre water.)Take paracetamol for fever only if tests are negative for Dengue. No aspirin/combiflams etc. should be taken for fever.7 days of rest is needed even after the body temperature has become normal, and it takes only 3 days to get normal from 104 with proper treatment.Don't take heavy medications unless indicated by a culture test (done rarely if Ampicillin does not work for Typhoid cases.)Wear cotton clothes.Try drinking- Weak tea with more milk and sugar.Clear soups.Roohafza in a glass of toned milk diluted with water in a ratio of 1:4, which means mix a glass of milk and 4 glasses of water with Roohafza.Remember gurupurab & roadside stalls offering Amrit to every passer by? This takes care of basic protein given through milk, at least 2 glasses of milk/day.Follow a no fat diet and rely more on liquid nourishment. Eat light food only (no fried food/maida/fats or heavier non-veg)I emphasize on taking adequate bed rest multiple times throughout this article since that's the most important thing people seem to miss. I've seen this mostly in people who earn daily wages because they are too scared to take a few days off to take bed rest.They will get back to work as soon as they feel their body temperature has lowered, which is a big mistake. This leads to exertional fever in these patients and they only get worse than before.We must educate all the people around us who do not have the luxury of reading information online and who are the ones worst affected by this hype that has been created.What can doctors do when they see a patient with 104 F ?Take a detailed history of shiverings, joint pains, swellings, rash, duration of fever, headache & relation of fever to Paracetamol (Yes, most patients do take few doses before coming to doctor.)Exclude Typhoid fever by doing a Widal Test (it costs 100-200 only.)Do a platelet count to rule out dengue. Malarial parasite may not be always seen on the slide, as it only comes out of the liver at specific times, when the person will have high fever with shiverings.Every foreigner coming for a visit to malaria affected areas is given 4 tablets of chloroquine as prevention.With a 104 degree fever, there's no harm in taking 4 tablets of Chloroquine after eating a packet of biscuits with tea made with more toned milk. It helps you get carbs, proteins, and water through tea.Chikungunya tests positive only on the 7th day and if treated well, it takes only 3 days to get well and be fever free. The joint pains & rashes take a few more days to fully heal.Check if the platelet counts are within normal range, perform widal test for typhoid. If that is negative, then it can be diagnosed by process of ruling out other illnesses and observation of symptoms.One can rule out Dengue/ Typhoid within 4 hours - yes good labs managed by MD doctors are giving excellent services through their collection agents within a radius of 10 kms.Try to take care of every fever patient at home first because there's very little need for hospitalization if proper care is taken at home.Do not get into politics of blame game between leaders, confusing junta about Chikungunya at Delhi. 4 people died at Sir Gangaram Hospital, but each one of them had actually landed at hospital with multi-organ failure. Why are we only blaming Chikungunya?

4 Signs You Should Be Aware Of, if Your Baby Has Fever

Being a paediatrician maximum of our OPD time is spent in treating fever related cases. Every parent must remember that not every fever is emergency. When a family welcomes a small baby in their family, along with the joy you invite many more things, few to name, responsibility towards the baby who cannot communicate, accountability towards every action towards the baby. And the most heartbreaking time is when baby is having fever with irritability.Here are few tips to help your child. You must have a thermometer at home, and fever medicine (paracetamol). Anytime you feel baby has fever, please document fever on a paper, as DATE/ TIME/TEMPERATURE. Help the baby cool down, by having minimum light clothing, give paracetamol as mentioned on the medicine box. Most important WIPE THE BABY FROM TOP TO BOTTOM WITH NORMAL TAP WATER with every fever spike, till temperature decreases to below 100FDanger signs indicating need to visit doctor are:1. Fever more than 101F2. Any symptoms other than just fever3. Altered sensorium4. Fever lasting more than 2 days. Visit your doctor in above cases and show her/him the temperature record maintained at home, which will help doctor make the right decision. Remember not all fevers require antibiotics to treat. In most fevers observation and symptomatic treatment is the only thing that is required. Can I Give Curd, Banana, Milk During Cold , Cough & Fever to My Baby?

Can I allow my child to play outside if he has a cough and cold ?This is a very tricky question since it largely depends on the weather and the intensity of the cold and cough of the child. If you think the child has a mild cold and the weather is amicable you can send your little one to play outside for an hour or so. Please also be aware that if he has an infectious type of cold or a cough he might infect his playmates as well (this is the most common reason for contracting cold in children)2. Can I give Bananas, grapes, watermelon, oranges during cold and cough to my child?It has been programmed into our brain that "No fruits during cold and cough in Children" and many of us follow blindly. This is a myth that has been proved wrong now.It has been proved scientifically that increase in fruits rich in vitamin C are in fact protective against cold and cough in children. CHIKUNGUNYA is a mosquito-borne viral disease.  It is an RNA virus that belongs to the alphavirus genus of the family Togaviridae. The name “chikungunya” derives from a word in the Kimakonde language (local Tazanian language), meaning “to become contorted”, and describes the stooped appearance of sufferers with joint pain (ARTHRALGIA).BREEDING SITES SymptomsMost people infected with chikungunya virus will develop some symptoms.Symptoms usually begin 3–7 days after being bitten by an infected mosquito.The most common symptoms are fever and joint pain.Other symptoms may include headache, muscle pain, joint swelling, or rash.Chikungunya disease does not often result in death, but the symptoms can be severe and disabling.Most patients feel better within a week. In some people, the joint pain may persist for months.People at risk for more severe disease include newborns infected around the time of birth, older adults (≥65 years), and people with medical conditions such as high blood pressure, diabetes, or heart disease.Occasional cases of eye, neurological and heart complications have been reported, as well as gastrointestinal complaintsOnce a person has been infected, he or she is likely to be protected from future infections.TransmissionThe virus is transmitted from human to human by the bites of infected female mosquitoes. Most commonly, the mosquitoes involved are Aedes aegypti and Aedes albopictus, two species which can also transmit other mosquito-borne viruses, including dengue. These mosquitoes can be found biting throughout daylight hours, though there may be peaks of activity in the early morning and late afternoon.MODE OF TRANMISSION OF VIRUSDiagnosisThe symptoms of chikungunya are similar to those of any other viral fever.Consult your doctor if you develop the symptoms described above and have visited an area where chikungunya is found.The virus may be isolated from the blood during the first few days of infection. Various reverse transcriptase–polymerase chain reaction (RT–PCR) methods are available but are of variable sensitivity.Serological tests, such as enzyme-linked immunosorbent assays (ELISA), may confirm the presence of IgM and IgG anti-chikungunya antibodies.TreatmentThere is no vaccine to prevent or no specific antiviral medicine to treat chikungunya virus.Treat the symptoms:Plenty of rest.Drink plenty of fluids prevent dehydration.Take medicine such as acetaminophen or paracetamol to reduce fever and pain.Do not take aspirin and other non-steroidal anti-inflammatory drugs (NSAIDS until dengue can be ruled out to reduce the risk of bleeding).

These Patients Have Felt Swellings on Feet, Days After Fever However, Only For Three Days

I am writing it from Delhi, now flooded due to incessant rains, epidemics of Dengue, Chikungunya, Malaria, Typhoid, Jaundice and other water /airborne infections. Humidity is 100% and droplets of water, not seen by us trap allergens, bacteria and viruses in them.The latest about Hepatitis B infection is Transmission from human to human through droplets in air (otherwise, it is through infected needles and body fluids unlike Hepatitis A which is a water borne disease)Viral infections like known / unknown are self-limiting and rarely require heavy antiviral drugs which are not specific. Commonly seen viral infections are Dengue and Chikungunya (it has nothing to do with chicken, infection was detected in an African country and name is local for the place where it was diagnosed first)Only Paracetamol 500 mg 8 hourly is needed for symptoms of joint pains & fever.Once Dengue is ruled out by blood report of good counts of platelets ( 1-4.5 lakhs), one can start Combiflam, and other NSAIDS for Joint pains seen and felt by patients of Chickengunea. Above medicines carry a risk of bleeding in cases of Dengue. Only paracetamol is safe if platelet counts are lower than 1 L.Lots of fluids are needed , besides total bed rest to save loss of water through sweat which will be there in any case once the fever comes down from 104 to 99 within a day provided no heavy foods are given to avoid vomitings and gastric disturbances.Do Not give plain water, it causes distention. Give only ORS - home made or sachets bought from shop.Coconut water is like plasma fluid - give this juice but stay away from other juices available in market on the road side or over the shelf.Mosquitoes breed in water and cause disease and Water is The Therapy !One can give clear soups, diluted toned milk for proteins and dry biscuits for wheat.But No Fats/ Maida/ Heavy spices/Heavy grains.Home made ORS- Juice of a lemon + spoon of sugar/honey/Roohafza + pinch of salt in 1L water.Diluted milk 1:4 with water with Roohafza to be sipped frequently. Chachh is another healthy choice as it has essential amino acids as proteins to repair damaged cells.Due to Fever or due to Water Retention, patients Do Complain of Rash. Apply No Creams/Lotions/gels etc. Keep the skin as dry as possible by being indoors. Apply medicated powder like Clotrimazole lavishly to affected areas ( available as Mycoderm-C / Absorb, looks like Nycil and costs the same ) Despite Every Caution , Despite fever being under control after 104 F, why are healthy adults complaining of swellings on feet as if there is water retention.No extra salts were given all through the phase of fever.Remember lots of breakdown of blood cells has occurred during the process of bringing down the fever.Most probable cause of swelling is temporary Anemia, and it is not due to Liver or kidneys as No Heavy antibiotics have been given during fever  to affect these vital organs.It is a Passing Phase.Reduce salt to bare minimum but do not eat a Bland diet after fever. Say No to packaged foods like Sauces , Pickles , Papads , Chutneys etc because salt with sodium is a Preservative and is Added to Every Packed food.Counter the effects of sodium in little salt taken by taking more Potassium Through Food Only ( no supplements as they can harm the heart but foods rich in potassium are safe)Take as much as you can -lemon, citrus fruits like oranges , mausami, bananas , raw amla but No Juices , only whole fruits with fiber which will absorb toxins causing distention and swellings on feet ( visible on feet but swellings are all over the body) Swelling around brain does make a person a bit dumb ,but it is just for the time being !Do not sit with legs with feet not supported , keep a stool under feet as a support.Get a massage done with movements towards the heart and not the other way .The Swellings are scary as they cause some load on the body too but there is No Need to take Diuretic medicines like Lasix to get rid of them.  Potassium rich foods plus drinking lots of water does help in removing extra water from body by inducing more urine generation the natural way and removing the collected toxins after the Fever
