Hello Doctor, I am experiencing severe weakness, sharp pain in my calf muscles, and lower back pain sometimes .My Hemoglobin, TSH, Blood Sugar, and Electrolytes are all completely normal. Could this be a Vitamin D3 or B12 deficiency, and which further tests or treatments do you recommend?"

Hello dear From the history, vitamin D/B12 deficiency are possible, but they should not be assumed to be the cause. Severe weakness with calf pain deserves a little broader evaluation like
25-OH Vitamin D
Vitamin B12
Serum ferritin + iron/TIBC/transferrin saturation

If you had recently fever episode this could be post viral fever symptoms. If you don't have any history of fever then you need to provide more information about your routine activities and can get your vitamin B12 levels and vitamin d3 levels checked. Can consult me for further guidance.
Normal Hb, sugar, thyroid and electrolytes are reassuring, but these symptoms need examination rather than assuming a vitamin deficiency.
I would check vitamin B12 and 25-OH vitamin D, along with calcium/magnesium and kidney function; calf pain may also be muscular or nerve-related.
Please avoid starting high-dose supplements without reports.
Next Steps
Please see a physician for calf and back examination. Seek urgent care if one calf is swollen, red or warm, or if you develop breathlessness or chest pain. You may book a Practo consult or visit Synergy Multispeciality Hospital, Miraj for assessment.
Hello. Severe weakness with calf muscle pain and intermittent low-back pain can have several causes, including vitamin D or B12 deficiency, iron deficiency, calcium/magnesium abnormalities, muscle or nerve-related conditions, and others. Normal Hb, TSH, blood sugar and electrolytes are reassuring but do not by themselves rule out vitamin deficiencies.
Treatment should ideally be based on the examination and test results rather than starting calcium or vitamin D blindly.
Next Steps
Consult a physician for examination and consider testing 25-OH vitamin D, vitamin B12, ferritin/iron profile, calcium, magnesium, phosphate and renal function. Seek urgent assessment if calf pain is unilateral or associated with swelling, warmth/redness, or if chest pain/breathlessness develops.
Hello. From the history provided, a deficiency of Vitamin D3 or Vitamin B12 is certainly possible given the symptoms of calf muscle and lower back pain, despite normal Hb, TSH, and sugar levels. However, a broader evaluation is recommended. You may consider getting tests done for 25-OH Vitamin D, Serum Vitamin B12, and a complete iron profile (Serum ferritin, iron, TIBC). Also, a detailed clinical evaluation including complete dietary and lifestyle history would be helpful. Please consult a doctor in person or via proper consultation with these reports for accurate diagnosis and prescription."

Next Steps

I would suggest testing Vitamin D, B12, calcium, magnesium, phosphate, and CK levels. A clinical examination is needed to rule out musculoskeletal or neurological causes.
Hello mam
Severe weakness and muscle pain can be related to vitamin D or B12 deficiency, but testing is needed before treatment. Consider checking Vitamin D, B12, calcium, magnesium and CK based on clinical assessment.

Does the pain worsen while walking?
Do you have any numbness or tingling in your legs?

Health Tips
don't do vigorous massaging over the calf area.
do emergency consultation if you have any symptoms of breathlessness, symcope or chest pain
Your Hemoglobin, TSH, sugar, and electrolytes being normal doesn't rule out a vitamin D or B12 deficiency. Those need their own specific blood tests and aren't picked up on a routine panel.
Next Steps
Ask your doctor for a vitamin D and vitamin B12 blood test. Low levels of either can cause exactly this pattern of weakness, calf pain, and back ache.
Health Tips
Please don't start any vitamin D or B12 supplement on your own before testing. The right strength depends on how low your levels actually are.
