Why Feces Isn’t Shit: The Brain in Your Gut

Dr. Sachin Chittawar, Endocrinologist
Am I completely out of my mind to literally call "shit" as "not shit"? The end product of our body which we get relieved off every morning is the excretion of our digestive system. We have seen people following cows while grazing for dung in India. The utility of cow dung is something we know is indisputable in India. Is there any value to human feces?Has human shit become a priceless thing for just about? Is it like kidneys for some who need a renal transplant?Has it got as important as heart patient waiting for a donor? It was Joshua Lederberg  who coined the term ‘microbiota'  the forgotten organ of the body. This breakthrough led to many scientific conclusions, making us believe that it is the gut which thinks like a brain. The relationship between gut flora and humans is not merely commensal (a non-harmful coexistence), but rather is a mutualistic, symbiotic relationship. The microorganisms perform a host of useful functions, such as fermenting unused food matter, training the immune system via end products of metabolism, preventing growth of harmful species, regulating the development of the gut, producing vitamins for the host (such as biotin and vitamin K), and producing hormones to direct the host to store fats.Immunity The natural commensals of the human gut are the bacteria which maintain the equilibrium between the good and bad ones in our intestines. They provide the template antigens for the immunity in our body. When we are frail, there is a dramatic fall in the gut flora. Healthy gut flora is becoming a marker of good health. The symbiotic relationship between a mammalian host and its gut flora has a significant impact on shaping the host's immunity. The immune system and bacteria engage in "crosstalk", exchanging chemical signals. This permits the immune system to recognize the types of bacteria that are harmful to the host and combat them while allowing the helpful bacteria to carry out their functions. A newborn gets the intestinal commensals from the mother's milk and some of it comes from the exposure to the external environment and their own feces. This early colonization helps to show the symbiotic microbiome inside early in its life. The bacteria are also able to stimulate lymphoid tissue associated with the bowel mucosa. This enables the tissue to produce antibodies for pathogens that may enter the gut.Diarrhea Gut flora maintains a proportion in the gut milieu. The balance helps us in fighting diseases like diarrhea. The good organisms decide the friendlier environment of the gut. Most of the trivial diarrhea episodes can go with a tablet of good bugs available in the marketplace. Pre and probiotics are the ones which create a good atmosphere for the wellness.Irritable bowel syndromeIn layman terms, the storm of disturbance in the gut does not settle in this disease. Illness revolves around the bowel disturbance. The tyranny of frequent visits and no relief continues. The pain can go by creating a good friendlier environment in the gut. Insulin resistance in type 2 DiabetesThe root cause of diabetes is insulin resistance which manifests in darkening of the neck, skin tags and the poor metabolic parameters in the conformation of the poor lipid profile with more of triglycerides and lowering the good cholesterol the HDL. The blood pressure goes up and the chances and the risk of having a future heart attack also increases. Research has proved that the gut bacteria are responsible for the upkeep of the insulin sensitivity. Gut decides the insulin sensitivity and governs the disease. Asthma, allergy, some cancers are the illnesses triggered by our deranged immunity and we can touch on it by creating a disease curing atmosphere in our gut. Extensive modification and imbalances of the gut microbiota and its microbial or gene collection are related to obesity. Good bacteria can be  a solution for curing obesity. Where to grow good bacteria? Gut microflora is principally composed of three bacteria: Prevotella, Bacteroides, and Ruminococcus. There is an affiliation between each bacteria and dirt. For example, Prevotella is related to carbohydrates and simple bread, while Bacteroides is associated with proteins, amino acids, and saturated fats. One organism will dominate depending on the diet. Intestinal milieu changes in diet. Trillions of bacteria reside in harmony in out the gut. They make 60% of our stools. Frail, ill people have less of the colonies and are more prone to suffer.  The best source is the milk and the milk products. Yogurt is a fertile source of lactobacillus lacti. Yeast is also a supplier of the bad bashing bacteria cabbage, cauliflower, beans, bananas and many plants and fruits are a good source of these good bugs for the gut. There are plenty of nonpharmaceutical products in the market in the form of nutritional supplements which are known to increase the good gut factors. Drugs delivering the good ones are also available. What should we do to protect gut- Avoid taking antibiotics irrationally Take antibiotics under supervision of a doctor Get the full course as advised Self-medication could be life-threatening. The physician should also think twice for prescribing antibiotics for trivial matters. Is the there anything like fecal transplantation?Like cow dung , human feces from a healthy individual has been used as a drug for those who are ill. This is in its preliminary stages. This also reassures the benefit of having good gut flora for a healthy body.

Is Your Baby Constipated ?

Dr. Kalpesh Onkar Patil, Pediatric Surgeon
Concerned about bowel function has been prevalent throughout history across many cultures. A normal bowel pattern is thought to be a sign of good health. Unfortunately, no uniform definition of childhood constipation is recognized. Moreover, healthcare providers have definitions of constipation that are very different from most parents' definitions.1) How frequent is the problem:-Constipation in children has reported prevalence rates between 1% and 30%.It is the principal complaint in 3-5% of all visits to pediatric outpatient clinics and as many as 35% of all visits to pediatric gastroenterologists.2) When is it called "Constipation":-The Paris Consensus on Childhood Constipation Terminology (PACCT) defines constipation as "a period of 8 weeks with at least 2 of the following symptoms: defecation frequency less than 3 times per week, fecal incontinence frequency greater than once per week, passage of large stools that clog the toilet, palpable abdominal or rectal fecal mass, stool withholding behavior, or painful defecation."3) How to deal with Constipation in Children:- Childhood constipation is treated in many ways, and virtually any therapeutic regimen is likely to be effective as long as it is sufficiently aggressive and persistent. Because of the medical profession's understanding of the patho physiology of the problem, the basic tenets of therapy include evacuation of the colon, elimination of pain with defecation, and establishing regular bowel habits.A group of patients with severe constipation that does not respond to conservative medical therapy requires more aggressive treatments, including surgery.4)  Diet:-Get educated about ideal feeds to baby, about constipating factors in foods, importance of daily water intake, importance of toilet training for your baby and how to set biological clock in their mind. YOUR FEW BASIC STEPS MAY CHANGE THEIR LIFE FOREVER"LET'S HELP THEM TOGETHER"

Do You Feel Pain When Passing Stool? (ANAL FISSURE)

Dr. Maruthesh Gowda, Bariatric Surgeon
What is Anal fissureAnal fissure is the most common cause for painful defecation (Opening bowels). Most times it is also associated with bleeding but not all the time. It is a small cut in the anal canal upto skin.Pain while passing motion which can last upto few hours afterwards is the commonest complaint. This is usually associated with Constipation (Passing hard stool/pushing hard to open bowel). At times it is seen after episode of loose motion. How is such severe pain caused?Easiest way to understand is, if you sustain a small cut while cutting vegetables, pain will persist till the cut healed and get aggravated when ever anything touches is -- including clothes. Similarly pain get stimulated when the stool passes through the anal canal touching the fissure.  What is the treatment of anal fissure? When treated appropriately in the initial stages, fissure heals upto 75% with medicines. Longer the duration less likely it is to heal with creams and might need simple surgery to relax the sphincters so that fissure can heal.Main stay of treatment includes keeping stools soft. This is usually achieved by laxatives during the period of anal fissure but in the long run by eating more fruits and vegetables, drinking at-least 3 lts of water in a day and exercise.Local anesthetic cream applied 20 mins before passing the motion will help in relieving the pain. Second cream helps to relax the muscle which helps in the healing of the fissure. With this treatment usually 50-60% of fissures heal. Next step should be injection of BOTOX into sphincters if patients are affordable. This will increase the chances of healing upto 75-80%. This is more so recommended in females as they have weak and small sphincters compared to men and more over they can also be damaged at the time of vaginal delivery. BOTOX injection causes only temporary relaxation of muscles (upto 6 months) so that fissure can heal.Other wise in patients in whom fissure does not heal by conservative management simple surgery is required. This will reduce the pain significantly within 24 hrs. What is Chronic anal fissure? Any anal fissure which has not healed even after 6 weeks is called chronic anal fissure. Conservative measures are unlikely to work and one might need surgery most of the times. What are complications of Anal fissure? A small skin tag (Sentinel skin tag) can develop in chronic fissures. Otherwise some patients can experience itching.Fissure will never lead to cancer.

Best Way to Treat Constipation in Diabetes at Home

Dr. Nikhil Prabhu, Diabetologist
Majority of diabetic people suffer from constipation, irregular bowel movements, abdominal cramps, dyspepsia, etc. Uncontrolled diabetes or Hyperglycemia affects nerves that supplies gastro-intestinal tract and hampers peristalsis. This leads to stagnancy of food in gastrointestinal tract. Diabetes also affects the nerves which supplies pyloric sphincter, causing disturbances in its relaxation, which ultimately leads to delayed gastric emptying. Nerves supplying to antrum of stomach are also affected leading to improper crushing of food and accommodation of food in stomach. This finally leads to dyspepsia & constipation. Normally, food we eat reaches rectum in around 36 to 48 hrs. In diabetic person due to imbalance in gastric motility this time span gets further delayed.6 ways to treat constipation in Diabetes at homeYou can treat constipation in diabetes with following six ways :Increase Fibre in diet – There are two types of fibre in diet (insoluble & soluble). High fibre diet helps to form bulk of stool, also increases water content of stool, makes it soften so it can easily pass through gastro-intestinal tract. Cereals, whole grains, beansare reach in fibre content. Green leafy vegetables, Fruits like apples, pears, berries etc are good for diabetic people. Breakfast with oats helps to get good amount of fibre at the start of the day.Drink lots of water – If you do not drink good amount of water, body ultimately tries to take water from food you eat & making stools harder. If you are increasing fibre in diet then increase water consumption as well. Drink atleast 1.5 litre of water daily.Regular Exercise – Regular exercise is necessary for all diabetics. It not only helps in controlling blood sugars but also helps to pass food further in gastro-intestinal tract. Even a moderate amount of exercise in the form of brisk walking or cycling helps to prevent constipation.Natural Probiotics – Daily a cup of yoghurt or a glass of Chass helps to keep healthy intestinal bacterial flora leading to good digestion of food and preventing constipation.Laxatives – Laxatives can be of different types like bulk forming, Osmotic, Stool softeners, bowel stimulatory. etc. Isbgol Powder and castor oil are commonly used home made laxatives. It is advisable to consult your diabetes doctor before starting any laxatives.Last but not the least Excellent Blood Sugar control with diet, exercise and regular timely medication plays and important role in treating constipation in diabetes. Cause of constipation can vary from Nerve damage of GI tract due to long term poor management of diabetes to just simple lack of fibre in diet.Extreme cases of constipation like Fecal impaction more commonly affecting elderly and disabled bed ridden individuals requires enema or manual evacuation of hard stools by inserting one or two gloved fingers in anus. Those were the Natural ways of treating constipation in diabetes at home.If you like my article please share it with your friends and family.Awareness is prevention!

Bowel Motion After Food Is It Normal?? Why Am I Constipated?

Dr. Maruthesh Gowda, Bariatric Surgeon
Quite a lot of people seek help complaining of having to go to toilet immediately after food. They induce inself-medication to prevent it mostly in the form of acidity medications. They worry they have problem of absorption/digestion. While one simple answer is its not serious.Unless you loose weight significantly then there is no problem with absorption or digestion. If you observe children then you will get the solution. Most children, when they are small go to toilet immediately after food and as adults we tell its wrong and over the time they stop doing it.Sensation to go toilet after eating food is normal. Its called Gastro-Colic reflux. When one eats food and stomach gets distended sensation is sent to brain to let it know there is new food in stomach and its going to need space. Normally brain makes space by emptying any waste in the colon which is from the food consumed day before so it in-turn sends signal to colon to empty waste. But as adults we train our body to ignore the signal and wait for appropriate time and place, and with time it stops sending the signal unless its absolutely necessary. In people who are facing the problem the Gastro-Colic reflux is exacerbated that’s all and we need persistently retrain our body if we wish to do so.One can be assured the motion which goes out is never the food that was consumed few hours ago. Its waste from the food consumed day before. What are the remedies for constipation? For one pass motion normally one shouldEat enough fiber: This is readily available in all fruits, vegetables and green leaves. Drink at least 3 liters of water/ dayExercise: One should not expect their intestines to be mobile if they lack physical activity. Exercise does not mean gym it could be simple walking. One should avoid medicines till above three natural requirements are met. Only if you done all the above and still constipated then you should use medicines. It is best to avoid medicines as otherwise to open bowels you will need medicine and any long term usage of medicine is not recommeneded.

Easy Relief Tips for Constipation in Children

Dr. Saurabh Kumar, Pediatrician
Constipation is a condition whenPassing motion becomes difficult or it’s not regularMotion is hard and there is a sense of incomplete bowel emptyingLong standing constipation may lead to many serious health issues like fissure, piles, urinary tract infection, fecal impaction [ fecal matter changes to stone like consistency]. And slightly less serious issues like decreased appetite, acidity, increased gas and abdominal distension. Though this blog is about children but whatever I am talking is relevant even to adults. How to relieve constipationI will be discussing this under four headingsDiet modificationAdequate water intakeMedicationProper positioning of the child while passing motion1. Diet modificationThis is the most important aspect of treatment, we have to add more fibre in the diet as fibre helps in holding water in the intestine thus leading to soft motion. These are the foods which are rich in fibre –- Beans. All beans are good, weather baked beans, beans like kidney beans or beans in salads.- Wholegrain and wholemeal. Skip white bread and pasta, look out for wholegrain and wholemeal on the labels.- Brown or wholegrain rice. White rice doesn't offer as much fibre.- Pulses. As well as beans, chickpeas and lentils are full of fibre. Try using whole dals than split ones as they give more fibre.- Nuts. Almonds, pecans, and walnuts have more fibre than other nuts.- Dried fruit. Like figs and dates are rich in fibre.- Bran based cereal and other healthy cereal options.- Porridge. Porridge is made from oats which are a great source of fibre.- Vegetable. All green leafy vegetables [spinach, methi, soya], beans [flat and round], lady finger, green peas are rich in fibre- Fruits. Raspberries, pears, apples, strawberries, chikku, banana are rich in fibre and these fruit should be eaten with the skin (except banana)2. Adequate water intakeWater is important to make the motion soft. So adequate amount of water should be consumed, neither more nor less. Will discuss about it in detail in some other blog. Just to give a rough estimation children [more than 2 yrs] need around 50 to 100ml /kg body weight of water.3. MedicationMedication has a role initially to relieve constipation and helps in setting up a routine of passing motion regularly. So discuss about it with a paediatrician.4. Proper positioning of the child while passing motionIf child is using western toilet then please make sure that his/her legs are properly supported on a stool instead of hanging mid air. This is important as this relaxes the child and helps in complete evacuation.

5 Ways by Which You Can Get Relief From Constipation

Dr. Amar Deep, Homeopath
Here are few tips to relieve constipation:1. Pure olive oil is more than just a healthy and tasty fat, it can also help relieve constipation. It stimulates your digestive system, which helps get things moving through your colon, and taken regularly it can prevent constipation as well. You will need… -1 tablespoon of olive oil and -1 teaspoon of lemon juice (optional)Directions -- In the morning consume one tablespoon of olive oil.It works best on an empty stomach, so have it before you eat anything else. If you forget, wait until later when you haven’t eaten for a while. You can mix it with a little bit of lemon juice if you like to lighten the flavor (lemon juice also acts as a natural aid for constipation.)2. The citric acid in lemon juice acts as a stimulant for your digestive system, and can also help flush out toxins and undigested material that may have built up along the walls of the colon. Mixing the juice with water not only lessens the intensity of the lemon flavor, but helps get you the fluids you need to get everything moving normally again. You will need… -1 fresh lemon and -1 cup of warm waterDirections -- Squeeze the juice of 1 lemon into 1 cup of warm water. Start slow and small here-if you don’t like the flavor of molasses, you can add it to a cup of warm water or tea to dilute the taste. If you find that a teaspoon has not been effective, you can go as high as 1-2 tablespoons, but try less at-first.4. Use pure aloe vera gel from the plant. The gel straight from the plant is more concentrated than commercial aloe juice so don’t use more than 2 tablespoons. If you don’t have an aloe plant, than drinking aloe juice can accomplish similar results. You will need…-2 tablespoons of pure aloe gel or 1 cup of aloe vera juiceDirections -- Mix two tablespoons of pure gel with fruit juice and drink in the morning, OR drink one cup of aloe vera juice as needed.5. Establish a routine -- Potty schedules aren’t just for puppies, they’re a great way to relieve and prevent constipation in humans too. Regulating the timing of when you go to the bathroom will regulate your bowel movements as well. Set aside about 15 minutes anywhere from 1-3 times a day, and take your time, even if nothing happens. Make sure you stay relaxed, and eventually your body will catch on.

Tummy troubles..not Any More!

Dr Trupti Shetty, Pediatrician
TIPS TO AVOID SUMMER DIARRHEADuring summers, extremely hot and/or humid weather conditions can result in severe gastroenteritis due to bacterial infection within the stomach and intestines. The most common causes of diarrhea during summers are infections caused by microscopic sized parasites named cryptosporidiosis and giardiasis which can spread from one another in humans and even from animal to persons by contracting illness from these agents. Diarrhea can be accompanied by other ailments like abdominal cramps, loss of appetite, nausea, mild fever, weight loss and vomiting. These diseases are particularly transmitted through contaminated food and water. However, to avoid these unforeseen circumstances during summers, it is advisable to follow certain natural preventive measures which include hygienic rules that ensure better chances of remaining healthy.Listed below are certain strategies which should be adapted to avoid the occurrence of diarrhea in summers:Wash hands frequently and properlySpead of infection caused by viruses, bacteria and parasites is very fast in the summer, aided by sweat which provides a good medium for the germs to multiply. To ensure safety from these, it is advisable to keep washing your hands or sanitizing them at regular intervals during summersConsider ProbioticsYogurt is a natural source of probiotics. It contains beneficial bacteria which help in restoring the microbial balance and prevent infection from reaching the intestines. Other sources of probiotics are green peas, dark chocolate, kimchi, fermentedpickles, soft cheese etc.Avoid under cooked foodsMake sure to cook foods like meat and eggs properly before consuming them as they may contain live parasites and bacteria which can result in chronic diarrhea conditions.Precautions while swimmingSwimming is a common cause of diarrhea during summers due to spread of infection caused by several viruses and bacteria which can contaminate water in the swimming pools. There viruses can be transmitted through a person’s rectum into the pool water and spread to others present in the pool. A person can transmit  infection in this manner for up to 2 weeks even after his recovery from the same. So, it is advisable to refrain from swimming for at least 2 weeks from the time you have been hit by diarrhea recently so as to protect others from getting it. As further precautionary measures, make sure to take a shower immediately after swimming and do not ever swallow pool water even by mistake. Mothers should be careful to wash the buttocks of small children carefully before descending them into the pool and never to change diapers near the poolside.Wash raw foods before eatingMake sure to wash rawfoods like fruits and vegetables properly and thoroughly with safe water before eating them as these are breeding grounds for development of bacteria which canresult in causing acute infections and ultimately diarrhea. Also, freshly cut salads and fruits should be consumed immediately. Remember you have to keep away from anything that can lead to the breeding of parasites and viruses which can cause diarrhea. Consume lots ofwaterSo that the toxins and any other kind  of infections present in the body are washed off naturally. Due to constant loss of water through sweat during summers, it becomes inevitable to constantly supply fluids to the body by consuming it orally. Also drink clean, filtered water as waterborne diseases are very common in summers.Opt for simple homecooked food. Avoid taking up diet fads, eating and drinking acidic foods and/or foods with high sugar content.
