Acidity:Acidity is a common name referred to regurgitation of acid or partly digested food from stomach into food pipe or mouth.Water Brash / Heartburn:Heartburn / water brash is burning sensation behind the breast bone or sternum. It is due to regurgitation of sour fluid from the esophagus into the oral cavity. It is normal when it occurs occasionally but frequent and severe heartburn is a manifestation of some esophageal dysfunction.Peptic Ulcer:Small or big erosion in the mucosa (lining) of the stomach.Causes:Stress and tensions. Emotional factors like fear, anger, tension etc. Overeating, large meals. Obesity. Alcohol,Cigarette smoking. Irregular eating habits. Certain drugs like, aspirin, antibiotics, pain killers etc. Bending, lying down or physical exertion soon after eating.After eating certain food like- onion, coffee, tea, tomatoes, citrus fruits, spicy, pepper, chilies, deep fried food, aerated drinks.In case of heartburn other causes include:Hyper acidity.Duodenal ulcer.Reflux of gastric contents into esophagus.Inflammation of esophagus, abnormal functioning of esophagus, distention of esophagus.Hiatushernia.Peptic Ulcer:Genetic factors Increased secretion of acid in stomach.  Blood group O. Smoking,alcohol. Druginduced- long term consumption of aspirin, antibiotics. Type“A” personality – aggressive, anxious, and ambitious. Prolonged stress and anxiety.Sign and Symptoms:Acidity:Sour eructation, belching. Burning in chest after eating. Vomiting of sour / undigested particles. A feeling that food is getting stuck after swallowing.Heartburn / Water Brash:Burning sensation in middle of the chest or upper abdomen. Aggravated after large meals, stooping or bending after eating. Spontaneous appearance of fluid in mouth, which may be salty, sour or bitter. Discomfort in stomach, aggravated by hot or cold foods, citrus juices and alcohol.Peptic Ulcer:Pains are related to meals. – Gastric ulcer- pain is worst after eating. Duodenal ulcer- pain temporarily better by eating so has to get up at night to eat again. Individual is healthy. In absence of pain – recurrent bouts of heartburn, anorexia, nausea and vomiting.Dietary Management:Treat the causes- take measures to relieve tensions and stress from life.Relax for about 20 minutes before and after eating. Eat bland food; less hot food add little or no oil to your food- bland boiled vegetables. Eat fresh fruits and salads, ash gourd juice. You can have takmaria, commonly known as falooda seeds (Impatiens balsamina), they have cooling effect and helps in digestion in hot weathers. Avoid citrus fruits, coffee, tea, raw vegetables, meat extracts, condiments spices and apple. Avoid canned and processed food- they contain chemical irritants. Avoid fried food and fats - you can include sesame oil, mustard oil, oily fish,linseed oil, skimmed milk and low fat milk products in your diet in small quantities. Avoid tobacco and alcohol. Eat meals at regular intervals, at regular time. Do not overeat; eat small meals at short intervals, little food at every 2 hrs. In snacks you can have biscuits (not the cream ones), toast with / without butter,sandwiches, light cake, curds, butter milk, ice creams, custards and puddings. Avoid drinking immediately before meals and drink sparingly during the meals. Do not exercise, bend, stoop / sleep immediately after meals.Avoid drugs like painkillers, antibiotics, aspirin, NSAID, steroids. Drink cold milk before retiring to bed.

Is It Really Acidity? Or Is it IBS?

I see many patients who come with self-diagnosis of acidity and self-medication. Most of them feel surprised when I tell them they actually not suffering from acidity. The fact is self medication is not only harmful butalso counter productive.What is Acidity?Burning sensation just below the chest bone in the centre either before or after food. If acid is coming upto chest then burning sensation in chest (heart burn) (Hiatus hernia/LAX LES will be discussed in separate blog). Some times it is so severe that you can confuse this with heart attack.Other symptoms like pins and needle sensation/burning sensation in chest/abdomen alone is probably not. Pain moving around different parts of abdomen alone is probably not. Back pain alone is probably not. These symptoms alone are not symptoms of acidity. These might be somatization of acidity if associated with symptoms discussed in the first paragraph. These symptoms alone are probably not diagnostic of acidity/GERD.What symptoms are not acidity?1  Feeling bloated, 2  Sounds in abdomen, 3  Rigid abdomen, 4  Pain in different parts of abdomen at different time5  Altered toiletry habits6  Feel full after eating little food. 7  Pricking sensation all over the abdomenAll these symptoms are not related to acidity at all. In fact, acidity medicines can make these symptoms worse. These are symptoms of irritable bowel syndrome (IBS).  This needs careful evaluation to rule out colorectal cancer in people who are over the age of 50 years. You may also need colonoscopy to rule out colorectal cancer if symptoms are new and you are above the 50 years.Generally this affects females above 40 years. Worrying problem is that it is now affecting young professionals because of their highly sedentary life style.These symptoms might also be caused from medicines and some diabetic medicines are most common cause. Poor control of diabetes aggravates these symptoms. Some people suffer from these symptoms after they had food poisoning (gastro-enteritis). This is usually self-limiting.  Some experience these symptoms after taking antibiotics for different conditions andagain this is also self-limiting.What are the home remedies for IBS?1  Drinking at-least 3 lts of water.2  Eating more fiber (all fruits, vegetables and greens)3  Eating less sweets4  Exercise (walking or any form of physical activity)While the above life style changes bring around life long relief of symptoms, medicines (Anti spasmodic/Pre& pro biotics) will help initially to overcome these symptoms. If you are suffering from above described you do not need medicines for acidity and in fact these medicines can make the symptoms worse.What food to avoid in Acidity?As such there is no prescribed diet for acidity. There is no need to avoid any food. Food habits leads to acidity not food items.Neither does spices cause acidity when consumed in limited quantity on time. If you put together the foods that lead to acidity that are listed by 100 people then there will be no items left to eat. So we should be able to eat all items. If  there is any particular food that increases the symptoms then avoid it, it is particular to that person.

Gastritis/acidity Part -2: Simple Ways to Manage Gastritis/acidity

Gastritis/ Acidity is usually mild and can be cured easily by following a better food habits and regular healthy lifestyle.However,sometime gastritis can cause pain in the upper part of the abdomen and may lead to stomach ulcer. Few over-the-counter (OTC) antacid medicines and a set of do’s and don't s can relieve gastritis..Some of the lifestyle and dietary changes are mentioned below:Lifestyle / Dietary changes: Small meals: Eat food in smaller quantity and switch to several small meals Avoid painkillers:Painkillers produces ulcers in your stomach and cause bleeding. So avoid un-necessary use of pain killers.Yoga & Physical exercise : Kapalabhati, Bhastrika and Anulomaviloma yoga increases movement of body and organs, helpful in the treatment of gastritis. Foods to be taken:Fruits: Fruits contain vitamins, minerals, fibres and antioxidants should be the part of the diet plan. Rice gruel:Rice gruel helps in the formation of watery gastric juice and reduces the acidity level within the stomach.  Only rice that has been aged for one year must be used. Wheat and barley are beneficial.Vegetables: Garlic and onion limits the growth of bacteria. Carrot juice, spinach juice, pumpkin, bitter gourd (Kaakarakaya),cucumber (Dosakaya), and green banana are effective. Broccoli is effective against H. pylori bacteria in the stomach lining.Dairy Products:The calcium and vitamin D in milk reduces the stomach acids and gives relief to the stomach but, it should be with low content fat. Grains:Whole wheat chapattis, pastas, noodles made from whole grains should be included in diet. Refined flour products should be strictly avoided. Limit the consumption of whole grain chapattis to about 2 chapattis per day.Green coconut water: Drink green coconut water, as it provides the stomach with the necessary rest, vitamins and minerals which provides an excellent and easy remedy for gastritis. Foods to be avoided:Salty and spicy foods.Vinegar and vinegar based food items.Aerated drinks, citrus juices, caffeine.High fat/cholesterol contained food items.Beef,fried fish and pork.Baked products like cookies, cakes, chocolates.Medical therapies: Several allopathic medicines are used for the treatment of gastritis, but for long time can produce disease like weak bones, constipation, vomiting etc. Antibiotics are used to kill H. pylori bacteria, which causes ulcers and gastritis present in your digestive tract. Antacids normalize acid production by blocking the action of the parts of cells that produce acids. Ulcer healing agents protect the tissues that line your stomach and small intestine. Besides these, surgery is preferred in complicated cases of gastritis like pus or mucous filled infections in stomach and intestines.Ayurvedic and herbal therapies: In Ayurveda, gastritis is known as Urdhavaga Amalapitta. It is caused in people who have pitta dosha, which means disturbance in digestion of food leading to indigestion. When the pitta dosha gets increased in such people, there are greater chances of getting gastritis. Ayurvedic preparations like Dhaatriloha, Sukumaraghrita, Sootashekhara rasa, Kaamadudha rasa, Leela Vilas rasa, Chandrakala rasa, Amalakichoorna, Triphala  choorna and Amalapittantaka rasa are beneficial in gastritis. If there is constipation along with gastritis, then Avipattikara choorna is taken as the medicine of choice. Several herbs beneficial in the gastritis patients:Cardamom (Ilaayachee) gives soothing effects on the stomach. It is especially beneficial if the gastritis causes heartburn.Fennel (Sauph) is digestive, traditionally used by Indians. When there is excess production of acid by the gastric glands of the stomach, it results in the condition known as acidity. 1. I am yet to come across any patient who might have come what ever reason not to complain that they suffer from acidity. This mean almost every body has self diagnosis of acidity which obviously can not be true.2. Everyone who has the self diagnosis has different home made remedy. If there are so many cures for  one condition surely one solution does not work for all.3. Most people self medicate with acidity tablets. If a study is conducted actual number of acidity medications sold in India might be more than lolly pops!!! This might be a fact.4. Foods that cause acidity. Different people list different foods. You do to have to listen to too many. If you listen the foods that need to be avoided from 100 people maximum then you will not be left with anything to eat. So it is plain obvious foods do not cause acidity. It also means you do not have to avoid any food on advise of others. 5. One thing is sure to cause acidity is food habits. As long the spice is with in normal limit and food is consumed on time there is no need to avoid any food. So as much as possible food needs to be consumed on time.6. Most important thing we need to understand is that acid is produced by everyone in the stomach so it is absolutely necessary  to digest food completely. Hyperacidity is a medical condition in which the stomach secretes a lot of acids.Causes of Hyperacidity: Medications: Hyperacidity can be caused due to prolonged intake of various medications. Stop Taking Ppi Group of Medicines Like Ranitidine as a Routine, Take Fennel Instead

PPI group of medicines are sold over the counter like sweets, people are taking it without realising the long term ill effects like liver & kidney failure and Dementia. Just about 20 years ago, as a routine some digestive enzymes like Unizyme, Aristozyme etc. were prescribed but these days, even people on antibiotics are given some PPI to avoid acidity ( may be few people have good digestion and they do not get acidity as a side effect, why burden their system with more medicines, besides emptying their wallets) Some of the routine names of PPI are Zenetec, Ranitidine, Pantaparazole etc. Rely more on home remedies and try to correct acidity symptoms through diet. Curd & Diluted toned milk are good ant-acids which give proteins and vitamins too.Everyone is oriented with their system, one should eat according to own Constitution.With age even our food pipe slows down but eating habits remain same. Do indulge occasionally but do not overburden the food pipe. Do Not Compare with others.
