Lichen sclerosis
Doctor, I have been suffering from Lichen Sclerosis for the past few years. Recently, I have been experiencing a lot of discomfort. The skin in the affected area has become very thin and fragile, and I frequently develop cracks and skin breaks. I also feel that the affected area has become narrowed, and I get swelling there from time to time. The symptoms improve temporarily with treatment but tend to recur. I am quite परेशान and worried about the repeated skin damage and narrowing. I want a proper and long-term treatment so that the condition can be controlled and, if possible, the narrowing can be improved. Please guide me regarding the appropriate treatment and whether the narrowing can be reversed.

I think your case needs a combined dermatologist obstreticians help along with psychological support. Will need to see what all current medications you are on and plan any change if required/ if any atypical skin change further evaluation by dermoscopy/ biopsy may be required.

Lichen sclerosus is a long-term condition, so symptoms often improve with treatment and return once it stops. That pattern is how the disease behaves, not a sign your treatment has failed. The thinning, cracking and narrowing happen from ongoing inflammation under the skin.
Next Steps
Topical steroid treatment usually needs to continue long after symptoms settle, so inflammation stays controlled. If the narrowing is affecting you day to day, a procedure can sometimes help once the skin is calmer. Please see your gynaecologist to review your current treatment and plan next steps.
