Vitamin B12 level of 178 pg/ml indicates deficiency. Start with Inj. Methylcobalamin 1000 mcg IM daily for 7 days, then once weekly for 4 weeks, followed by once monthly for 3–6 months. If mild symptoms and good absorption, Tab. Methylcobalamin 1500 mcg daily for 3 months can be used after injection course or as maintenance.
Next Steps
Recheck vitamin B12 level after 3 months. Evaluate for associated folate deficiency and thyroid or intrinsic factor issues if persistent low levels. Take a balanced diet with milk, eggs, meat, and fortified cereals.
Health Tips
Prefer injection route if you have tingling, numbness, or weakness. Avoid alcohol and smoking, which impair absorption. If taking metformin or antacids, long-term B12 supplementation may be needed.

Your B12 level is quite low. You can start Methylcobalamin 1500 mcg daily for 3 months with Folic acid 5 mg daily for 4 weeks. If symptoms like tingling or weakness are present, injectable B12 gives faster recovery. Please consult me online so I can choose the best form and dose for your condition.

ChatGPT said: Hello, I am Dr. Romain Rajan, General Physician. I understand your concern. Your vitamin B12 level of 178 pg/ml indicates a clear deficiency. This can lead to fatigue, tingling, numbness, poor concentration, and low mood if not corrected properly. ✅ Advices • Start Methylcobalamin 1500 mcg tablet once daily after breakfast for at least 3 months. • If you experience severe fatigue or tingling, you can take Methylcobalamin 1000 mcg injection intramuscularly daily for 5 days, then weekly for 4 weeks, followed by once monthly for 3 months. • Include foods like eggs, fish, chicken, milk, paneer, and curd in your diet. What to Monitor • Watch for improvement in energy levels and reduction in tingling or numbness. • Avoid alcohol and unnecessary antacids, as they reduce vitamin absorption. Management You may also take Folic acid 5 mg once daily for one month along with B12, as both work together for nerve health. Continue the supplement for at least 3 months and then repeat your B12 test.
