There is a broken instrument in the mesial root, it's possible that this could be contributing to the ongoing swelling and discomfort. It's important to address this situation as soon as possible to prevent any further complications. Here are some steps you might consider: Contact Your Dentist: Reach out to the dentist who performed the root canal treatment or a specialist to discuss your symptoms and the issues you're facing. They will likely want to examine your mouth and potentially take new X-rays to assess the situation. Possible Treatment Options: Depending on the severity of the issue, you might need to have the broken instrument removed and the root canal treatment properly completed. In some cases, additional procedures might be necessary, such as endodontic surgery. Consult a Specialist: If your current dentist is not able to address the issue, consider seeking a consultation with an endodontist, who specializes in root canal treatments and related procedures. Pain and Swelling Management: In the meantime, your dentist might recommend pain management strategies and possibly prescribe antibiotics to control the infection. Applying cold compresses to the affected area could also help reduce swelling. Remember, it's essential to consult a dental professional for proper diagnosis and treatment recommendations tailored to your specific situation. Dental issues can become more complex if not addressed promptly, so seeking professional guidance is crucial.

You need an examination by an endodontist. There seem to be a separate instrument in your mesial canal of the tooth. If the swelling is related to the RC treated tooth you may require a Re-RCT! With the whole side of your face involved you should not be delaying your treatment. Buy the cause of your swelling needs a physical examination before racing on the correct diagnosis.
Next Steps
Visit an endodontist

Not very clear from X ray. But as per symptoms,  you may need Re-RCT. Do visit a local Endodontist for physical evaluation and treatment (as required).
