This looks like a frozen shoulder. This is very common in diabetics and age above 40+ and females are more prone than males. So for the condition like this we do hot water fermentation on the affected shoulder front and back 10 mins each. Avoid sleeping on the affected shoulder. Avoid jerks on the shoulder as it can aggrevate pain. Pendular exercise are good for early recovery.
Next Steps
shoulder mobility exercises once pain reduces with heat.
Health Tips
do shoulder mobility exercises after heating only. any exercise u feel is creating more pain, avoid doing so.

You show typical signs of frozen shoulder. This condition is more likely to happen in people with diabetes. I advice you to take a good physiotherapy rehab for your shoulder. The early you start your physio rehab the better ..
Health Tips
avoid sleeping on your left shoulder you can apply hot/cold pack ; whichever helps you relieve the pain .. but it will only be twmporary.. exercise is the only solution.

Left shoulder pain could be because of your diabetes. I have few questions like How long you are diabetic?, Is your glucose level under control? Are you able to do over head activities? What is shoulder range of motion? As of now your symptoms shows it could be periarthritis shoulder because of your high glucose levels. You can use Volini spray or gel to the affected site provided you are not allergic. You can give hot water fomentation for 10-15 mins twice a day. Note: While giving hot water fomentation keep moving the bag on affected site to avoid burns.
Next Steps
For further evaluation and treatment you need to consult a Physician or Physiotherapist or Orthopaedist as early as possible.
Health Tips
Don't lie on the left side while sleeping. Avoid carrying any bags or small weight on your left hand. Avoid pulling and pushing actions with your left hand.

Dear sir Is your diabetes in control ?  And how long you are suffering from diabetes ? And Long term complications of diabetes may include changes in connective tissue that occur as a results of high glucose levels sir.
